Dr. Childebert St. Louis, Medical Director

Dr. Childebert St. Louis, MD is a family medicine specialist and offers complete health care for individuals and families. He has over 22 years of experience in the medical field. He graduated from Autonomous University of Tamaulipas / Faculty of Medicine in 2002. Dr. St. Louis speaks English, French, Creole, and Spanish.

Regina Cataldo, PA

Regina Cataldo, PA received her undergraduate degree in basic medical sciences from Fordham University 1988. She then earned her Master of Public Health Degree from NYU Medical College in 1995 and her Physician Assistant Degree from St Vincent/St. John’s Catholic Medical Center in 2006. She is a member in good standing of AAPA and NCCPA.

Wayne Chan, AGNP

Wayne Chan is a board certified Adult Gerontology Primary Care Nurse Practitioner with a holistic approach to care. Educational background includes a Bachelor's degree in Psychology from Kean University, and a Master's degree in Nursing from Russell Sage College. He is credentialed with St. Peter's Hospital, Samaritan Hospital, and Sunnyview Acute Rehab Hospital and affiliated with the American Association of Nurse Practitioners and Sigma Theta Tau International Honor Society of Nursing. He is also an adjunct professor, clinical instructor, preceptor, and on the NYS Board of Nursing and education committee. He completed a geriatric fellowship with rotations in palliative care, hospice, urology, cardiology, geropsychiatry, wound care, physical medicine and rehab, subacute rehab, and long term care.

Dr. Nidhi Dugar

Dr. Nidhi Dugar is a compassionate internist and primary care doctor. She obtained her medical degree from Kasturba Medical College Manipal and completed her residency in internal medicine at Flushing Hospital Medical Center.

Patients visit Dr. Dugar for various concerns, including alcoholism, allergic cough, anemia, halitosis, and asthma. She speaks English and Hindi fluently, which makes her culturally attuned to patients with various backgrounds.

Dr. Dugar is polite and patiently guides the ailing individuals through the treatment process. She aims to empower her patients and make them feel optimistic about their treatment plan.

John Giangreco, MSN, FNP-BC

John Giangreco, MSN, FNP-BC, is a family health nurse practitioner with more than ten years of nursing experience. John received his Associate of Applied Science in nursing from the College of Staten Island, New York, in 2010. In 2013, he went on to complete a Bachelor of Science in nursing from State University of New York at Delhi in Delhi, New York. Finally, in 2017, he received a Master’s of Science in family nurse practitioner from Wagner College in Staten Island, New York. Throughout his nursing career, John has gained valuable experience working with patients in the fields of emergency care, medical-surgical care, surgical step-down, and post-op care. He has excelled at working closely with patients to develop personalized treatment plans that empower men, women, and children to take charge of their health and wellness. John offers compassionate preventive care, competent diagnosis, and holistic treatments for his patients.

Terri-Ann McLean, FNP

Terri-Ann McLean, FNP is a board certified Family Nurse Practitioner with over 20 years combined nursing experience. After serving 4 years active duty as a combat medic in the United States Army Terri-Ann received a Bachelor’s degree in nursing from The College of New Rochelle. She returned to The College of New Rochelle’s holistically based nursing program to earn a Master’s of Science degree in Nursing. Her career experience includes emergency medicine, acute home care, long term care and primary care. Terri-Ann believes healthcare is a partnership between patients and their providers, and that providing compassionate care, can and should always be coupled with competent care. This philosophy is reflected in each of Terri-Ann’s patient encounters, in which she offers respect for individual preferences, patience in providing information and dedication to her delivery of care.

Dr. Maria Noya

Dr. Maria Noya is an internal medicine specialist and has over 49 years of experience in the medical field. She graduated from University of Santiago / Faculty of Medicine in 1974. She speaks English and Spanish.

Dr. Alec Pruchnicki

Alec grew up in New York City, attended public schools, Fordham college for his bachelor’s degree and did basic science research on aging at the City University of New York before medical school. He graduated from Albert Einstein College of Medicine in 1985, completed an Internal Medicine residency at Overlook Hospital in New Jersey and then a fellowship in geriatric medicine at UCLA. Since returning to New York in 1990 Alec has worked in a variety of in-patient and out-patient practices and for the last twenty years has had a primary care geriatrics practice at a community-run, non-profit, Medicaid supported assisted living facility in Manhattan.

Amna Rajpoot, PA-C

Amna Rajpoot, PA-C is a licensed Physician Assistant currently working for Visiting Docs managed by Health Heros. She graduated from SUNY Downstate Health Sciences University with a master’s degree in Physician Assistant. She is looking forward to work in family medicine, obstetrics and pediatrics.

Safi Sayah, PA

Safi Sayah is a Physician Assistant. He graduated from York College in Jamaica, Queens in 2012, and received his Master’s Degree from Touro University in 2020.
